I do prior authorization for Injection in office and ambulatory facility by submitting through the portal like Evicore, provider portal, UHC portal, Cohere health and by calling insurance to check if prior auth is required. I also make sure that I will gather all the information before submitting the request and check the CPT codes and ICD-10 are correct to get approval.
