About EZ Scribe LLC:
EZ Scribe LLC provides clinical documentation and compliance support to U.S.-based home health agencies. Our focus is ensuring accurate, audit-ready documentation that meets Medicare and CMS regulatory standards.

We are building a high-level Quality Assurance (QA) and coding division to reduce denial risk, improve documentation quality, and ensure full compliance across all client charts.

Role Overview:
We are hiring a seasoned OASIS Coder to work remotely from the Philippines. The primary focus will be coding OASIS assessments for U.S. home health clients; however, familiarity with quality assurance processes is required to provide coverage when needed. Only applicants with proven OASIS coding experience in the U.S. home health setting should apply.

Key Responsibilities:

-ICD-10 Coding: Review Start of Care, Resumption of Care, Recertification and Discharge OASIS assessments and assign accurate ICD-10 codes that reflect patients’ diagnoses and support proper reimbursement. Ensure coding aligns with PDGM and Medicare requirements.
-Plan of Care Verification: Confirm that orders, disciplines, visit frequencies and durations are properly specified and consistent with physician instructions, as required by CMS.
-Documentation Review: Check that OASIS data is accurate and complete—identifying mistakes in functional scores, risk of hospitalization data and other fields. Provide concise feedback to clinicians.
-Coding Compliance: Stay up to date on OASIS-E2 guidelines, PDGM updates and CMS Conditions of Participation. Maintain thorough, HIPAA compliant documentation.
-QA Support (as needed): Be prepared to perform quality assurance reviews on clinician notes and therapy evaluations. QA tasks may include verifying timeliness and completeness of records, ensuring therapy evaluations are comprehensive and include functional status, safety, home environment and patient/caregiver issues, and providing feedback when documentation does not meet standards.
-Collaboration: Communicate effectively with U.S. clinicians, coding auditors and QA tea ---------- mbers to resolve coding questions and documentation deficiencies.

Qualifications:

-Experience: Minimum 2 years of direct experience coding OASIS assessments for U.S. Medicare certified home health agencies. Experience in QA or clinical documentation review is a plus.
-Certification: Valid coding certification (CPC, CCS, HCS-D or equivalent). COS-C certification or strong OASIS-E/E2 proficiency preferred.
-Clinical Knowledge: Deep understanding of Medicare Conditions of Participation, PDGM, OASIS guidelines and plan of care requirements. Ability to recognize documentation errors and ensure data integrity.
-Education: Licensed health care professional (RN, PT/OT/SLP) or graduate of an accredited medical coding program.
-Technical Skills: Proficient with home health EMR systems (e.g., DataSoft Logic, WellSky, AxisCare), Google Workspace and secure file sharing.
-Language: Excellent written and spoken English; able to produce clear, detailed comments and reports.
-Work Environment: Remote; requires reliable high speed internet and a secure workspace. Willingness to work during U.S. business hours.
-Compliance: Must sign a Business Associate Agreement and strictly adhere to HIPAA privacy and security rules.
